Dear Sir/ Madam,
I beg to state that, I am a student of (Your Class), (Your Section). With due respect, I would like to apologize to you because of my absence from your class for the last 3 days and for not attending the class test.

I caught on a high fever. I was very ill for the last 5 days. I consulted with a doctor. He prescribed me some medicines and suggested bed rest. That’s why I can’t attend the last 3 classes and class tests. Here I provided my blood test report and my prescription.

You told us that the class test was very important to us. It would contain 20 marks which will add to our final exam marks. As you know I am a quite meritorious student. Last semester, in your subject, I got the highest marks. You praised me. You also encourage me to study well. Now in the current semester, I can’t take the risk to lose 20 marks. So, please sir accept my sincerest apology and give me a chance to do the class test under your supervision.

May I, therefore, pray and hope that you would be kind enough to allow me to do the class test and oblige thereby.
